DURING THE TOUR
The social worker will explain the levels of care and services available within our program, discuss how the cost of services are covered, and review the admission process and necessary paperwork.
HISTORY AND PHYSICAL/HEALTH RECORD FORM
This form is needed to begin the enrollment process. Click below to download. We will also need a current TB screening completed by the potential participant's primary physician.
FACE TO FACE EVALUATION
Your social worker will help coordinate a face-to-face evaluation with the applicant's managed care provider.
HOME VISIT
Once the completed documents have been returned, the social worker will arrange a home visit with the participant and caregiver.
SCHEDULE A START DATE
Upon approval, the social worker will schedule a start date and hours of attendance based on family/participant need and availability.
RETURN COMPLETED DOCUMENTS
Once signed and completed, all forms are returned to the social worker.
REVIEW
IEADHC/CBAS Center Health team will review the information collected by the social worker to decide if the program meets the potential participant's needs. The managed care provider determines the days of attendance based on medical and social needs of the participant.
TRANSPORTATION
The Center will review transportation options and assist with applications if necessary.

“What a blessing the Inland Empire Adult Day Health Care Center has been to my mother and our family. As a respected and educated woman in the community, my mother was diagnosed with early-onset Alzheimer's and has been attending the center for over 3 years. Attending Monday through Friday every day from 8am to 1:30pm. Because of the love, education, and support the staff gives my mother, she can thrive in the late stages of her Alzheimer's disease. She gets socialization, activities, exercise, a meal, and is being monitored by the nursing staff. For my mother's involvement at the center, she has been able to live longer and have a better quality of life. The staff is the utmost professional group of women.
